The basics
An MtF hair transplant uses follicular unit excision (FUE) to move individual follicular units into a redesigned feminine hairline. Each unit is taken one by one from the back and sides of the scalp. The new line sits lower where appropriate, rounder in shape, with the temple angles softened. Unlike the older strip method (FUT, follicular unit transplantation), there is no linear scar. We would rather tell you plainly: FUE leaves tiny dot marks in the donor area, though they are very hard to find once healed.
Transplanted follicles come from a zone naturally resistant to DHT (dihydrotestosterone), the hormone behind male pattern loss. So they keep growing in their new position for the long term. However, no clinic can honestly promise a fixed outcome. Most suitable patients are in good health, settled on their hormone regimen and have enough donor hair to meet the design requirements. Oestrogen and anti-androgens usually slow further loss, but they rarely restore a hairline that has already receded. Grafting does. If the plan is unrealistic for your donor supply or the timing is wrong, he will say so.
OUR APPROACH
Feminising a hairline is a design problem before it is a surgical one. Three measurements do the work: the height of the line, the roundness of its shape and how far the temple angles close. Dr Bonaros draws the design with you at the mirror, then builds a leading edge of single hairs at irregular intervals. Each graft is angled to match how your hair naturally falls. The person who plans the line is the person who places every graft.
Your donor area is finite, and a feminising hairline is graft-hungry, so the plan protects that reserve. It allows for a second pass if density calls for it, and for how your remaining native hair may behave in the years ahead. You are quoted for the grafts your plan needs, not the most that can be billed.
WHY PATIENTS CHOOSE FUE
Each follicular unit is placed by hand to set its angle, depth and direction. The new hairline curves softly rather than starting as a hard edge. Transplanted hair continues to grow over the long term, though individual results vary.
It is carried out under local anaesthetic with no stitches and no linear scar. You go home the same day. For suitable candidates, an unshaven FUE approach keeps longer styles intact while the grafts heal.
Most patients are back at a desk within five to seven days. Recipient-area redness typically settles over two weeks. Donor marks usually close within 10 days.

THE PROCEDURE
Everything starts with a consultation, in person or online. Dr Bonaros examines your scalp and donor area and discusses how the timing of surgery fits your wider transition. The proposed line is agreed with you until the height and shape feel right. You leave with a realistic graft estimate and a fixed written quote setting out exactly what it includes. Nothing changes on the day. One rule of preparation is absolute: never stop prescribed medicines, including hormone therapy, without your prescriber's advice.

2. Local anaesthetic is placed: a brief sting, then numbness for the rest of the day.
3. Follicular units are extracted one by one from the donor area.
4. Recipient sites are made along the drawn line, setting the angle, depth and direction of every graft.
5. Grafts are placed individually, with single hairs leading, and you head home with written aftercare instructions.
6. Most sessions run six to eight hours, depending on graft numbers, breaks included.
Grafts anchor within the first few days, and any scabbing lifts by around day 10 to 14, between weeks two and eight, much of the transplanted hair sheds. This is shock loss. It alarms almost everyone, and it is expected rather than a setback.
New growth starts around months three to four. The change becomes obvious by month six, and the result matures at 12 to 18 months. Our FUE recovery timeline walks through each month. Temporary swelling, itching or numbness is normal. Serious problems are uncommon, and every risk is discussed with you before consent. You are reviewed at set points across the year, with photographs tracking progress.

Hormone therapy is not a formal requirement, but timing matters. Many patients choose to be settled on a stable regimen first. Oestrogen and anti-androgens usually slow ongoing male pattern loss and make the long-term design easier to plan. Dr Bonaros discusses your timing openly at consultation, and where it is sensible to wait, he will say so.
It usually will not. Hormone therapy tends to slow or partly stabilise further male pattern loss, and some patients notice modest thickening in thinning areas. A hairline that has already receded rarely returns on hormones alone. Transplanting DHT-resistant follicles into the recession is a reliable way to rebuild it.
Yes. Minoxidil can thicken existing hair for some patients, and where assessment supports it, prescription treatment to lower DHT may help protect native hair. However, many trans women already achieve that suppression through their existing regimen. These options support the hair you still have rather than rebuilding a receded hairline, and none should be started or stopped without your prescriber's advice.
It depends on how far the line moves and how far the temples close. Many MtF hairline plans fall somewhere between 1,500 and 3,000 grafts, and larger designs are sometimes staged across two sessions to protect the donor area. Your number comes from the examination, not a menu.
Often, yes. For suitable candidates, Dr Bonaros offers an unshaven FUE approach. Only small sections of donor hair are trimmed and hidden beneath the surrounding length. Longer styles remain intact throughout recovery. Suitability depends on graft numbers and hair characteristics, and is confirmed at consultation.
The one routinely uncomfortable moment is the local anaesthetic: a brief sting, then numbness. During the procedure itself, most patients read, watch films or doze off. Afterwards, tightness or mild soreness for a few days is normal, and simple pain relief is usually enough.
Transplanted follicles are taken from a donor zone that is naturally resistant to DHT, and they keep that resistance in their new position. So growth is long-lasting for most patients. Native, non-transplanted hair can still change over time, which is exactly why the design plans for the years ahead. Individual results vary.
FUE leaves tiny dot marks scattered through the donor area rather than a linear scar. Once healed, they are very difficult to spot, although healing differs from person to person. If you have a scar from previous surgery, camouflage grafting may be possible and can be assessed at your consultation.
Yes. Many patients plan hair restoration alongside facial feminisation surgery (FFS), and sequencing matters. Forehead or brow surgery is generally completed first, so the hairline design and any scar camouflage can respond to the final position of the skin. There is no price list, because no two designs need the same work and case complexity varies significantly. After your examination, you receive a fixed written quote for your specific plan. It states exactly what it includes, typically calculated at around £4 per graft, and does not change on the day. The quote reflects the grafts your design needs.
